Patellar impingement with femoral condylar plate

28 year old Salesman 8 month post orif of Intercondyalr fracture femur. Presented with ROM 10 to 80 degrees ans pain on extension.
O/E. decreased patellar mobility, Clicking band(Plica) lateral to patella at 20 to 10 degrees of extension. Cruciates and collateral stable.  views please

  1. scopic arthrolysis with lateral release. The fracture seems to be united. removal of the plate and the lateral screw projecting out to be removed at the same sitting afetr the arthrolysis.

  3. I would scope. release the gutter and lateral release after a lateral facetectomy remove any interfering screws. floowed by mua and lastly plate removal.

  4. is it time to take condylar plate of the shelf. or do it right. this is the thrid case of patellar damage seen in my carrier. the pllate needs to be more posterior.

  5. but sir, is the plate to be blamed fully for such instances? this seems to be an issue with distal femur LCP too...especially when done as MIPPO for diaphsyeal and metaphyseal porotic fractures. trusting C arm pictures alone plus the porotic nature of bones causes anterior position of plate and also at times locking screw entering the femeropatellar joint. so i do mini lateral arthrotomy as the distal incision of MIPPO instead the longitudinal incision
